What do I do if my check is Lost, Stolen, or Expired?

Did you know?   According to the Social Security Administration, physical checks are 16 times more likely to be lost or stolen compared to electronic payments?

If you have not received your check and have waited fifteen (15) business days from the date the check was originally mailed, please complete the attached Lost Check Affidavit (attached below) and return to Urban:

Urban will verify the status of your check with our Accounting Department and stop payment if the check is still outstanding. The funds will then be reissued under a new check number in accordance with our pay policy.  Funds from your expired, lost, or stolen check will be added to your next royalty payment and may include funds for current payment.   A separate check is not reissued.  Please allow up to 60 days for your Lost Check Affidavit to be processed. 

Urban offers Direct Deposit, which helps mitigate fraud due to lost, expired, or stolen checks. Urban encourages all owners to sign up for direct deposit to avoid these situations. Please complete and return the Direct Deposit Form along with a Voided Check.

If your check is over 90 days old, it will not be honored. Urban is not responsible for any fees you may incur from your bank by attempting to present a Stale Dated Check for payment.
